Upgrade to Micrometer 1.0.0-rc.3

Upgrade to Micrometer 1.0.0-rc.3 and refactor existing
auto-configuration to align with updated APIs.

Note that Spring MVC instrumentation has now changed from an interceptor
to a Filter.
